MS Athletics Information

All required paperwork to participate in athletics at Mayfield Middle School must be completed through Final Forms (FF_ParentPlaybook.pdf). * Please note that Mayfield-issued student email addresses CANNOT be used as they do not accept outside emails. Students will need to register using a private email address. The email address used MUST be different than the parent's email address.
